I, the parent/guardian of the registrant, a minor, agree that the registrant and I will abide by the rules of Chasing What Chases You (CWCY). Recognizing the possibility of physical injury associated with basketball, and in consideration for CWCY accepting the registrant for its programs and activities, I hereby release, discharge, and/or otherwise indemnify CWCY, its affiliated organizations and sponsors, their employees, and associated personnel, including the owners of fields and facilities utilized for the programs, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the programs.  

EMERGENCY MEDICAL AUTHORIZATION: I hereby give my consent to have an athletic trainer, coach, and/or doctor of medicine or dentistry provide my son/daughter with medical assistance and/or treatment and agree to be responsible financially for the reasonable cost of such assistance and/or treatment.

Registration Fee: A non-refundable fee of $350 is due at the time of registration to secure the athlete’s roster spot and order custom gear (uniforms/kit).

Monthly Tuition: Following the initial registration, a monthly payment of $170 is due on the 1st of each month for the remainder of the season.

Total Season Cost: The total program value is $1,200.

Late Fees: Payments received after the 5th of the month will incur a $25 late fee.

Refunds: Because CWCY is a 501(c)3 non-profit that commits funds to tournaments and gear in advance, all payments are non-refundable. If an athlete leaves the program mid-season, the remaining balance for gear already ordered may still be due.
